A large ornate pseudo-Gothic-style plaque on a black limestone backing. The brass is ornately decorated with an ogee-shaped pedimented arch with a crocketed finial. This is flanked by shields with the letters Alpha and Omega surrounded by foliage. Between the cusps of the arcaded pattern are the O'Hara achievement of arms and the motto "TRY". The IHS monogram occurs in a roundel on the pediment. Roundels bearing the symbols of the Evangelists occur in each comer. The maker's name "COX & BUCKLEY, LONDON" is stamped in the lower right hand comer of the plaque. The lettering and decoration are inlaid in red and black enamel.
